You are responsible ensuring that all the ESXi hosts in a large vSphere environment are kept up-to-date on patches and updates. You use VMware Update Manager (VUM) to facilitate the update process. All of your ESXi hosts are running ESXi 5.0 or later. You reply on the Update Manager Download Service (UMDS) to automatically download updates from the Internet.
